Application for Facility
Use and Waiver
If you would like to reserve a room to use for an activity, you
may open a copy of the Application for Use and the Waiver form below.
The office must receive a Facility Use Application from each sponsor who would like to reserve space in the Ministry Center, the Potter’s House, the Auxiliary Classrooms, or the grounds. The procedure for requesting use of specific space for your event is:
- Call Pat at the office (Tuesday through Friday, 9:00 a.m. to 2:30 p.m, 410-730-6004) to see if the space you’d like to use is available on the date and at the time you desire.
- Complete a Facility Use Application form and give it to the office. This form is available by clicking either the Adobe Acrobat or Microsoft Word link below and also in the black Congregational mailbox on Sunday mornings. You may email it to the office (pat@cpc-opc.org), mail it, or drop it off. The space will not be reserved until the application is received.
- Pat will schedule your room.
- Pat will email you confirmation.

in detail in the CPC Facility Procedures Manual above.
